Abstract: PT. Sinar Sanata Electronic merupakan salah satu perusahaan dalam proses pembuatan bola lampu. Dari proses pembuatan bola lampu PT. Sinar Sanata Electronic terdapat beberapa kesalahan pekerja. Dari kesalahan yang dilakukan oleh para operator seperti kelebihan atau kekurangan waktu dalam proses pencetakan bola lampu, yang mengakibatkan tidak sesuai nya hasil produksi yang diharapkan dikarenakan terlalu lama atau cepat sehingga menyebabkan produk tidak sesuai standard. Adapun tujuan dalam penelitian kali ini untuk mengetahui faktor apa saja yang menyebabkan human error dan human error tertinggi dengan uraian produksi dalam proses pembuatan bola lampu ialah, proses pembentukan bola, proses steam, proses pemasangan filament, proses celling, proses vacum, proses penyatuan base cap dengan bola lampu, proses penyolderan, proses Quality Control, proses pengemasan.sehingga penulis melakukan Penelitian dengan judul Analisis Human Reliability Assessment dengan Metode Human Error Assessment and Reduction Technique. Untuk proses ini dilakukan tahapan awal yaitu menetapkan HTA dan hasil tersebut akan menghasilkan 9 task dari tahap awal pembentukan bola lampu sampai proses pengemasan. Berdasarkan nilai perhitungan terdapat nilai HEP terbesar pertama pada proses Quality Control dengan nilai 0,5556 dan nilai tertinggi kedua pada proses penyatuan base cap dengan nilai 0,1339. Kemudian dengan hasil ini penulis menganalisis jenis pekerjaan dengan menggunakan Diagram Fishbone. Kemudian penulis melakukan upaya pencegahan Human Error berdasarkan HEP, EPC dan Fishbone Diagram. PT. Sinar Sanata Electronic is one of the companies in the process of making light bulbs. From the process of making light bulbs PT. Sinar Sanata Electronic has several employee errors. From errors made by operators such as excess or lack of time in the process of printing light bulbs, which results in not conforming to the expected production results due to taking too long or quickly causing the product to not meet standards. The purpose of this research is to find out what factors cause human error and the highest human error with production descriptions in the process of making light bulbs, namely, the ball forming process, the steam process, the filament installation process, the celling process, the vacuum process, the base cap unification process. with light bulbs, the soldering process, the Quality Control process, the packaging process. so the authors conducted a study entitled Analysis of Human Reliability Assessment with the Human Error Assessment and Reduction Technique Method. For this process, the initial stage is to determine the HTA and these results will produce 9 tasks from the initial stage of forming the light bulb to the packaging process. Based on the calculated value, there is the first largest HEP value in the Quality Control process with a value of 0.5556 and the second highest value in the base cap unification process with a value of 0.1339. Then with these results the author analyzes the type of work using the Fishbone Diagram. Then the authors make efforts to prevent Human Error based on HEP, EPC and Fishbone Diagrams.
