Abstract: Kesalahan manusia di UKM. Kotama Shoes terdapat beberapa jenis cacat yang sering terjadi yaitu kerutan pada bagian toe dan heel sepatu,dan tidak merekatnya bagian atas dan sol sepatu yan terlihat celah kecil. Pada penelitian ini bertujuan untuk mengetahui tingkat probabilitas human Error pada proses produksi, mengetahui faktor yang mempengaruhi human error operator pada stasiun perakitan dan memberikan usulan perbaikan untuk operator stasiun kerja. UKM. Kotama Shoes merupakan salah satu perusahaan dalam proses pembuatan sepatu kulit. Human Reliability Assessment dilakukan dengan menggunakan metode HEART (Human Error Assessment and Reduction Technique). Pada tahapan awal dilakukan analisis terhadap tugas yang harus dilakukan operator dengan menyusun HTA (Hierarchical Task Analysis. Berdasarkan perhitungan diperoleh tingkat probabilitas human error terbesar terdapat pada stasiun perakitan yaitu dengan Assessed Effect 18,816 dan selanjutnya pada stasiun penjahitan yaitu dengan Assessed Effect 7,6653. Kemudian menganalisis faktor penyebab human error pada stasiun perakitan yang memiliki nilai assessed effect tertinggi menggunakan fishbone diagram. Kemudian menyusun upaya pencegahan human error berdasarkan EPC, HEP dan Fishbone diagram yaitu memberikan edukasi dan pelatihan bagi operator, menerapkan penjadwalan/shift dalam bekerja, memperbaiki tata letak stasiun perakitan, menambah jumlah operator pada stasiun perakitan.
