Abstract: PT. Adil Bersama Indra adalah bengkel pemeliharaan tabung (Retester tabung) gas LPG 3 KG. Terjadinya human error dalam pengecekan tabung gas di PT. Adil Bersama Indra, hal ini terlihat dari pengembalian tabung yang telah dikirim ke SPBE, adanya tabung bocor body, bocor valve, berat kurang dari standar dan beberapa jenis kerusakan tabung lainnya, yang disebabkan kelalaian operator (human error) yang dilakukan oleh operator, yaitu tidak memperhatikan tekanan angin, tidak mencelupkan tabung dengan benar, tidak memperhatikan berat tabung, kesalahan dalam menulis penambahan plat tabung, dan tingkat kedisplinan yang rendah. Untuk melihat tingkat keandalan manusia (Human Reliability Assessment) dilakukan dengan menggunakan metode Human Error Assessment and Reduction Technique. Tahapan awal menyusun Hierarchical Task Analysis, berdasarkan hasil pendiskripsian pekerjaan dalam pengecekan tabung gas yaitu 14 task jenis pekerjaan yang dilakukan oleh operator. Berdasarkan perhitungan diperoleh nilai HEP setiap task, kemudian peneliti menganalisis jenis pekerjaan dengan nilai HEP yang tinggi menggunakan diagram pareto. Terdapat empat task yaitu pekerjaan Pengisian tekanan angin 8 kg/cm2 dengan nilai HEP sebesar 1,742, pekerjaan Leak Test akhir dengan memberi tekanan 8 kg/cm2 dan mencelupkan tabung ke bak dengan nilai HEP sebesar 1,496, kemudian pekerjaan memperhatikan berat penimbangan tabung nilai HEP sebesar 1,344, dan pekerjaan Pemeriksaan Valve dengan memberi tekanan udara 12 kg/cm2 nilai HEP sebesar 1,210. Kemudian peneliti menyusun upaya pencegahan human error berdasarkan EPC, HEP dengan Fishbone diagram. Faktor metode kerja yang masih perlu pelatihan, kedisplinan mengikuti SOP, faktor lingkungan yang lebih aman dan nyaman, faktor manusia yaitu pendidikan dan usia pekerja, dan faktor alat dan mesin. PT. Adil Bersama Indra is a repair shop for 3 KG LPG cylinders. The occurrence of human error in checking gas cylinders at PT. Adil with Indra, this can be seen from the return of the tube that has been sent to SPBE, the presence of leaking tube bodies, leaking valves, less than standard weight and several other types of tube damage, which were caused by operator negligence (human error) by the operator, namely not paying attention to air pressure, not dipping the tube properly, not paying attention to the weight of the tube, errors in writing the addition of the tube plate, and a low level of discipline. To see the level of human reliability (Human Reliability Assessment) is carried out using the Human Error Assessment and Reduction Technique method. The initial stage is compiling a Hierarchical Task Analysis, based on the results of job descriptions in checking gas cylinders, namely 14 tasks of types of work carried out by operators. Based on the calculation, the HEP value for each task was obtained, then the researcher analyzed the type of work with a high HEP value using a Pareto diagram. There are four tasks, namely the work of filling the wind pressure of 8 kg/cm2 with a HEP value of 1.742, the final Leak Test job by applying pressure of 8 kg/cm2 and dipping the tube into the tub with a HEP value of 1.496, then the work paying attention to the weight of the tube weighing the HEP value of 1.344, and Valve Inspection work by applying air pressure of 12 kg/cm2 with a HEP value of 1,210. Then the researchers compiled efforts to prevent human error based on EPC, HEP with Fishbone diagrams. Factors of work methods that still need training, discipline following SOPs, environmental factors that are safer and more comfortable, human factors, namely education and age of workers, and factors of tools and machines.
