Abstract: PT. Pacific Medan Industry is a company in Medan which receives Foreign Capital Investment and engaged in cooking oil processing, Samin oil, shortening, margarine, and bottle packaging.Based on the data of the company, the 1-liter bottle production has a high defective enough.The defective 1-liter bottle often occurs were broken, oblique, leaked, and nonstandard-color bottles.The research aimed to discover factors that causing the defective on the 1-liter bottle. Six Sigma method was used in the research with DMAIC (Define, Measure, Analyze, Improve, and Control). Then, every DMAIC phase was conducted by analyzing and maintaining every 1-liter bottle production. The result revealed that the defective quantities on the 1-liter bottle are 52,604.1667 defect per million opportunity (DPMO) with stigma level of 3.12. Moreover based on Pareto diagram, the most defective level on the 1-liter bottles were broken bottle of 54.85 % ; oblique bottle of 38.71 % ; nonstandard-color bottle of 3.96 % ; leaked bottle of 2.48 %. Furthermore based on fish bone diagram (cause-effect diagram) showed the causeof defectives on the 1-liter bottle. They were less control in raw material, less optimally engine maintenance, and prioritizing operator experience. Thus, some recommendations in the research are to increase engine maintenance and cleanliness, to set a standard work procedure and instruction, and to give training to operators in the production process and engine production maintenance routinely, also to give training to employees in the engine control.
Description: PT. Pacific Medan Industri merupakan salah satu perusahaan di kota Medan yang mendapatkan Penanaman Modal Asing (PMA) dan bergerak di bidang pengolahan minyak goreng, minyak samin, shortening, margarin, dan kemasan botol. Berdasarkan data perusahaan, produksi botol 1L memiliki kecacatan yang cukup tinggi. Kecacatan botol 1L yang paling sering ditemukan yaitu botol pecah, botol tidak berdiri tegak (miring), botol bocor, dan warna botol tidak standart. Penelitian ini berfokus untuk mengetahui faktor penyebab kecacatan pada botol 1L. Metode yang digunakan dalam penelitian ini adalah six sigma dengan DMAIC (Define, Measure, Analyze, Improve, and Control). Setiap langkah DMAIC dilakukan dengan menganalisa dan menjaga setiap proses produksi botol 1L. Hasil penelitian ini menunjukkan jumlah cacat botol 1 L yaitu 52.604,1667 defect permillion opurtinity (DPMO) dengan tingkat sigma 3.12. Berdasarkan diagram pareto tingkat kecacatan botol 1L yang paling dominan yaitu botol pecah (54.85%), botol tidak berdiri tegak (38.71%), Warna botol tidak standart (3.96%), dan botol bocor (2.48%). Berdasarkan fishbone diagram (cause-effect diagram) dapat diketahui faktor penyebab kecacatan pada botol 1L. Faktor-faktornya yaitu kurangnya pengawasan terhadap bahan baku, kurang optimalnya dalam pemeliharaan mesin dan mengutamakan pengalaman operator. Sehingga dapat disarankan beberapa rekomendasi yaitu untuk meningkatkan perawatan dan kebersihan mesin, menetapkan prosedur dan instruksi kerja sesuai standart, dan memberikan pelatihan kepada operator dalam proses produksi dan pengendalian mesin produksi secara rutin dan memberikan pelatihan kepada karyawan dalam pengendalian mesin.
